4. Describe the service(s) that clients are receiving now and what changes are needed (based on their needs from the outcome of the client engagement)
Question 2
1. Describe the new program you would like to implement
2. Explain what other services, networks and stakeholders you consulted to develop and implement the program
3. Explain how the new program would impact on the current organisation service delivery
4. Discuss what individual differences, rights, needs and preferences you need to incorporate into the planning process. Also discuss what support are needed for effective consumer participation
5. Meet with clients to discuss the new program and ask for input on how to implement it. Submit a summary of these discussions and client suggestions
6. Identify what financial, human and physical resources will be required to implement the new program
7. Discuss what systems and processes will need to be changed/modified to accommodate and implement the new program 8. Develop and submit a feedback form that you will give customers in your new program to evaluate the new program and ensure client satisfaction.
Two feedback forms are required, these are to be use for evaluation
9. Explain how you would use the feedback
10. Discuss how the program will be funded (fee for service, fundraising, donors, government funding, etc.)
11. How will you monitor the budget?
12. What standards, codes and legislation do you need to comply with when develop and implement this service program?
