Scenario: A 4-year-old boy, Riley Jacobs, with a diagnosis of asthma has been admitted through ED and given rescue puffs to manage an asthma attack. He has required 3 hourly Ventolin and has been admitted overnight. His mother is staying with him. His SaO2 is 95% on room air and his BP, temperature and HR are within normal limits. His RR is around 35-38 when Ventolin is required. In order to be discharged in the morning, Riley's wheeze must have settled and he must be comfortable and managing with Ventolin at least 4 hours apart. The mother is anxious to stretch his doses of Ventolin to 4 hourly, however she has expressed to all nurses and doctors that she must be at work in the morning. He has been given a dose of oral prednisolone and has been written up for another dose in the morning. The mother has anxiety and depression and has advised that she does not have her medications with her.
