The patient is a 50-year-old male with a history of dyslipidemia and osteoarthritis who presents with several weeks to months of epigastric abdominal pain and dyspepsia. His pain is described as burning and improves with food, suggestive of a gastric etiology such as peptic ulcer disease (PUD). He reports past episodes of black specks in vomit and black, tarry stools, raising concern for prior upper gastrointestinal bleeding. Contributing risk factors include daily NSAID use (ibuprofen), low-dose aspirin, moderate alcohol intake, and a 30-pack-year smoking history. Physical exam reveals epigastric tenderness without peritoneal signs. His review of systems is otherwise unremarkable, and there is no evidence of current bleeding. While the patient has not had prior GI workup, his risk profile and symptom pattern suggest the need for evaluation of ulcer disease or gastritis, possibly with H. pylori testing and EGD.
