Matthew is a 23-year-old single white male who presented for the first time to his local mental health center. He was a recent college graduate who had just moved to the area to begin work in a small accounting firm. His chief complaint was insomnia of five years duration. He stated that the insomnia was due to fee lings of guilt, anxiety, and emptiness; the reasons for these feelings he could not identify. There was no history of any previous mental health treatment. He reports being in "several relationships and I guess I become too much to be with." He tells you he does "everything I can to keep them happy but they always abandon me." He reports multiple suicide attempts over the past 3-4 years, has been a "cutter" most recently 2 weeks ago. MSE is within normal limits.
