1. Explain the cause of this patient's difficulty in maintaining her balance? Diazepam is a benzodiazepine which causes central nervous system (CNS) depression which can cause symptoms of drowsiness and loss of coordination (Rosenthal and Burchum, 2021).

This patient having difficulty maintaining balance could be related to a higher than expected serum concentration, considering the elderly population experiences age related changes in the organs, changes in cellular receptors that impact the absorption, distribution, metabolism and excretion of drugs (Rosenthal & Burchum, 2021). Along with this, chronic use with possible lack of long term management and drug education, further drug accumulation in the body and diazepams' long half life can all play a role in this adverse effect.

2. Diazepam experiences a significant first-pass effect. What is the first-pass effect, and how can first-pass metabolism be circumvented? The first pass effect is the rapid metabolism in the liver of some oral medications resulting in inactivation of the medication (Rosenthal and Burchum, 2021). When an oral medication is swallowed it is absorbed into the gastrointestinal tract, to the liver and then to systemic circulation where it takes effect.
