Urinary Elimination Case Study

Clyde Hunter, a 72-year-old male, is a resident of a long-term care facility. He has been unable to control the urge to void since experiencing a stroke. Prior to his stroke, Clyde often awakened 5 or 6 times during the night to void but was able to control the urge long enough to make it to the bathroom. Since the stroke, Clyde now voids spontaneously without recognizing the need to void.

1. Identify significant symptoms that indicate the client has a health problem

2. What additional information do you need to gather to create your care plan?

3. List at least 2 nursing diagnoses (patient problems) for Mr. Hunter. What is your highest priority? Justify your answer
