First, I would check with the patient that he understands what information is being presented to him; that he understands ALL of the information. If he is unable to coherently understand, then the patient obviously cannot sign the document. In a respectful tone, I would explain to the surgeon that the patient does not understand the consent form he is signing. You have observed the patients slurred speech and that he is having difficulty hearing. You have attempted to make sure that he understands the document and his impairment continues to impede his understanding. It is also best that you make sure you understand the facility's policy on conflict of interest and being a witness to a patient signing an operative permit. The hospital more than likely has policies in place that could put your career in jeopardy, should you agree to go along with the surgeon. The patient is top priority and always comes first.
