A 25 year-old male, 12 hours postoperative a thyroidectomy is complaining of tingling around mouth, difficulty speaking and difficulty swallowing. Neck dressing has a 1 cm in diameter blood stain. The nursing assessment reveals: cold clammy skin, muscle twitching of the face, irregular breathing pattern and frequent attempts at swallowing. The report given by the first shift nurse indicated that the patient has been somewhat irritable, refusing diet and restless at times. VS BP-130/70, P-90, R-22, IV lactated ringers @ 100 ml/hr. STAT electrolytes and CBC ordered. What could be the problem (s)? What should you assess? What should you do?
