Social determinants of health can have a major impact on a person's health when they don't have access to safe housing, education, job opportunities, income, healthy food, transportation, physical activities, neighborhood, language and literacy, water, etc all of these environmental conditions can affect a person's quality of life (Healthy People 2030). Social determinants of health contribute to the development of a disease when a person not having access to healthy food choices due to (low income, neighborhood, or education) it causes that individual to develop health conditions such as obesity, heart problems, and a shorter life expectancy from poor nutrition diet (Healthy People 2030).

The communicable disease chain model demonstrates how the process of disease is spread (Jacob and Cummins, 2019). The communicable disease chain has six links called the causative agent, infectious reservoir, path of exit, mode of transmission, the path of entry, and susceptible host (Jacob and Cummins, 2019). The causative agent is usually a virus, fungi, bacteria, or parasites and a micro-organism that can cause an infection (Jacob and Cummins, 2019). An infectious reservoir is when the infection can spread and grow and allows the pathogen to multiply it can spread to humans, animals, and inanimate environmental objects such as environmental surfaces, floors, walls, medical equipment, furniture, etc. (Jacob & Cummins, 2019). The path of exit is the pathogen leaves the reservoir to grow such as a respiratory tract infection. The mode of transmission is how the infection spread for example in the hospital infection can spread from direct physical contact or indirect contact with an object that is contaminated, or it can be airborne such as Tuberculosis just by breathing in the particles (Jacob and Cummins, 2019).

The nurse can break the link within the communicable disease chain by disposing of contaminated needles in a sharp container to prevent the infectious agent to get to the host and by washing your hands before and after leaving the patient room to break the chain mode of transmission from direct touch (Chain of Infection, n.d.).
