Social Determinants
Social determinants are environmental conditions that affect where individuals are born, live, work, worship, and age that affect the health and quality of living. There are 5 domains in social determinants: economic stability, education access and quality, health care access and quality, neighborhood and built environments, and social and community context. Social determinants contribute to the development of disease (Green, 2018). In an environment, an individuals who is living in a rural community can not have access to a grocery store with healthy food. The diminished resources of healthy foods can lead to certain disease and illness. Individuals who have economic stability are more likely to be healthy. These individuals are able to afford healthy items, housing, and health care (U.S, 2020).
Communicable Disease Model
The communicable disease model explains how a disease is passed from one individual to another. The passing of this disease can be prevented at any time. The three sections of this model are infectious agent, host and environment. The infectious agents consist of bacteria, viruses, and parasites. The host is an organism that can be invaded by the infections agent (Chain, 2020). The environment is any environment that can host or promote the transmission of the disease.
Breaking the Chain
The nurse is able to prevent the steps or break the chain. Hand washing is the first step in the preventive measures that nurses can teach their patients. During the assessment phase a nurse can help administer vaccinations to stay up to date. In contact with patients who have certain contact and airborne disease, the nurses uses preventative measures such as putting on a gown, mask or other PPE to prevent becoming a host of a disease and passing it to other patients. Nurses use standard precautions when coming in contact with all patients. Using gloves and hand hygiene in and out of the room is the first line of preventative measure that nurses uses during their encounter with a patient.
