A client sought treatment at a hospital clinic was prescribed co-trimoxazole (Bactrim), a sulfa drug for an infection. The client had a sulfa drug allergy; however, this fact was noted in the client's written chart, but not on the computer-generated record the resident reviewed before prescribing the drug. A day after taking the first dose, the client became ill and sought treatment at another hospital. She died 16 days after a severe allergic reaction to co-trimoxazole. The client's family sued the first hospital for the negligence of the discharge nurse, the pharmacist, and the medical-records clerk. It could not be found where the discharge nurse provided instructions about signs and symptoms of adverse reactions to medications and what to do.
