Bob was referred to you because he is struggling with a substance use disorder (alcohol and oxycontin). Bob meets the criteria for a severe substance use disorder (SUD), according to the DSM-5 (American Psychiatric Association, 2013). He began drinking and taking pills after a traumatic car accident, to help with the pain and to quiet the flashbacks. He has tried to quit numerous times on his own and is feeling a great deal of shame and guilt alongside describing himself as a failure. He does not understand why it is so hard to quit and is blaming himself for the SUDs. Bob does not understand how counseling and medication work together.
