Question

A 50-year-old male noted a swelling in his neck. His physician referred the patient for a thyroid sonogram. The work-up revealed moderate enlargement of the left lobe of the thyroid gland measuring 1.1 x 2.2 cm, a solid nodule at the anterior aspect of the middle of the left lobe of the thyroid measuring 1 x 1.9 cm; normal right lobe of the thyroid. Thyroid scan showed cold nodule, which is usually suspicious for malignancy. The patient is now referred to an Endocrinologist who performed an ultrasound guided fine-needle aspiration of the thyroid gland in the office today. The cytology report was read as Papillary with mixed follicular carcinoma of the thyroid.
