Question

A patient with an elevated prostate-specific antigen (PSA) of 35.7 comes to the surgery center for a transrectal, ultrasonic-guided (TRUS) prostate biopsy.

Technique: The patient is placed in the Sims position with the left side down. The anus was generously lubricated with 2 percent Xylocaine jelly. The ultrasound probe was then introduced and scanning initiated. A great deal of calcification was noted in the outer margin of the central zone. The area proximal and anterior to the calcifications was hypoechoic but may have been influenced by the stones. There was very thin peripheral zone tissue available. Three needle biopsies were taken from each side, starting in the periphery and working toward the midline and trying to biopsy anterior to the stones on the more medial biopsies from each side.

The pathology report confirmed carcinoma in situ of the prostate.
