Question 

A 24-year-old male was admitted due to motor-vehicular accident. The patient sustained multiple abrasions on the extremities and trunk. Patient medication orders by the hospitalist team include Acetaminophen 300mg IV, every 4 hours, as needed for pain and Amoxicillin 500mg IV 3 times per day. On the second day of admission, the patient complained of headache and stuffy nose. The patient's mother noted that the patient had nasal congestion a day prior to the accident and had taken Sinutab 1 tablet, as needed. The nurse referred the concern to the hospitalist. The hospitalist ordered Sinutab every 6 hours for his nasal congestion. On the 4th day of admission, the hospitalist gave a discharge order. As the nurse was giving discharge instructions to the patient and his mother, the patient suddenly complained of right abdominal pain, and then vomited. Discharge order was deferred. The following day, the nurse noted that the patient was manifesting jaundice. The patient also verbalized that his abdominal pain has been intermittent, and that the severity is increasing. This prompted the hospitalist to review the MAR and progress notes. It was then revealed that the patient had received a total of 3,200 mg of acetaminophen per day,
