Client complains of sharp, intermittent, severe pain in his right flank that radiates towards the groin. Client describes the pain as "stabbing" 10/10 intensity when it occurs, although currently pain is 2/10. He reports that the pain has been worsening over the past two days with increased discomfort during urination. Medical history includes gout, historically managed with a low-purine diet. The client states that due to recent financial constraints, he has been consuming more processed foods high in animal protein and salt. Client has no other past medical history besides gout, no allergies, and does not smoke. Client AAOx4, denies vomiting, diarrhea, and headache. Client states he does feel nauseous when the pain comes. Right CVA tenderness noted. Lungs clear to auscultation bilaterally. Cardiac auscultation demonstrates normal sinus rhythm with no rubs, murmurs, or gallops. Abdomen soft, nontender, nondistended with bowel sounds auscultated in all four quadrants. Neuromuscular system intact in all four extremities, no edema, full ROM. Client sent for labs this morning prior to visit. In-office point of care urinalysis (UA) shows trace hematuria, pH 5.9, specific gravity 1.010, no leukocytes or bitrates.
