Question

Write a nursing note in paragraph for a patient Respiratory support was being provided through a high-flow nasal cannula. A saline lock is in place on the right arm with a size 22-gauge catheter; the site is free of redness or swelling. Additionally, a right subclavian porta-cath is in place but currently not accessed. Notable findings upon examination include ecchymosis on both arms and swelling in both lower extremities. Monitoring and further evaluation of the patient's respiratory status and vascular access sites are recommended. Pt seen and examined 1/25. In bed. Awake, answering questions. C/o pain in back of moderate intensity, He did not answer further questions to characterize pain. He states he is thirsty. He asked for water which he was given. He tolerated few sips. Breakfast tray was brought in by staff. However, Pt refused stating he does not want food. He has mild to moderate sob. He is currently not agitated. He asked for pain meds. He was encourage to ask for pain meds as needed for symptom control.
