Roger presents to the office today. He is a 43-year-old patient under the care of Dr. Vixon. Dr. Vixon asked that I consult in this case as Roger has had left lower quadrant abdominal pain, on and off for two weeks. Patient is also diabetic and has hypertension. Dr. Vixon manages both those conditions but asked that I evaluate the LLQ pain. The patient's history is that he reports the pain will come and go. He does have hematuria and dysuria. Exam: Upon physical examination the patient has left flank tenderness. I obtained a urinalysis and upon review there was occult blood with one to five white blood cells, no bacteria present. Diagnosis: Renal Colic Plan: Prescription for Lortab, as needed for pain and he is sent home with a urine strainer for any stone he might catch. Patient is instructed on how to use the urine strainer and asked to return with any stone for further work-up and evaluation. Patient will return before then if condition worsens and a full report of my findings will be sent to Dr. Vixon.
