When you walk in the patient's room to begin your morning assessment, patient appears restless and agitated. The patient has a history of afib (atrial fibrillation) controlled by medications, DVTs in the recent past, and renal insufficiency. VS are as follows: T98.8, P132, R28, BP 100/78.
