Question

One clinical client-focused quality measure currently being analyzed in my practice location is process. The data is collected from documentation, OASIS, and care coordination notes. The primary staff looking into this is our director of nursing and the assistant director of nursing. This data is looked at to ensure that care, such as admissions, are being completed promptly. Data is run through a quality assurance program to ensure that information is accurate as well as being reviewed to ensure the plan of care is being followed. If there is an error, the documentation is sent back for further clarification or correction. After reviewing the information, the Director of Nursing determines where there are lapses or inconsistencies in the care. This way, the process can be improved to ensure patients have the best possible outcomes and needs are being met promptly. For advanced practice nurses, it is important to be involved in the quality measures process. They are often impacted by changes and can see how the outcome will affect the patient. The challenge they may face is not being considered in the process and being forced to advocate for their importance in this area.
