Home health nurse is performing an admission assessment on a 50-year-old male client with a left tibia/fibula fracture who has been ordered to be on crutches and non-weight bearing on the left leg. Nurse notices floor rugs and clutter on the floor. Nurse provides education and practice on the appropriate way for the client to utilize their crutches. Yesterday, 1200 Client calls the home health nurse and reports a fall with no significant injury. Client reports they do not need to be evaluated in the emergency department (ED). Nurse plans to return to re-evaluate client tomorrow. Today , 1100 Nurse returns to reassess client. Client states they have had "another fall today." Nurse notes no physical injury or bruising to client. Nurse requests to watch client use their crutches.
