Postoperative Diagnosis: Same. Operation: Primary low cesarean delivery. Indications and History: Valentina is a 30 y/o, G2, with a history of recurrent severe variable decelerations in the active phase of labor, who was consented for a primary urgent cesarean delivery. ANESTHESIA: Epidural anesthesia and general by endotracheal tube. ESTIMATED BLOOD LOSS: 1000 mL. DRAINS: Urinary catheter. Fluids: 2800 mL. Urine Output: 335 mL. Specimens: Placenta to Pathology. Complications: Vertical extension of Pfannenstiel incision. Condition: Stable. Description of Operation Patient was identified, and procedure was verified. The patient was taken to the operating room, where she was given a bolus of her epidural anesthesia. She was in the dorsal supine position with a leftward tilt. SCDs were present. A scalpel was used to incise the lower abdomen transversely. The underlying skin incision was carried to the level of the rectus sheath using a Bovie instrument. The Bovie was then used to nick the fascia in the midline and the fascial incision was then extended laterally using Mayo scissors. Kochers were then used.
