I recently had a 5 day old male new born brought into the clinic to 20 y/o african-american/hispanic mother for well-baby visit. Infant was born at full-term and via spontaneous vaginal delivery. At the hospital, the mother was treated prophylactically for maternal group B streptococcus infection. Utox was positive for opiates and fentanyl due to maternal intrapartum morphine and fentanyl. At the clinic just looking at the mother showed she was under the influence of something however the bond between the newborn and mother was beautiful to watch - calmed the baby by holding and talking to the baby. The mother was hands on with the newborn, denied any frequent times of sadness.

When the mother walked in she specifically asked to see my preceptor. She didn't want anyone to hold her baby. To me that showed how protective she was of her baby. She later felt comfortable and made me and another collegue NP to take care of the baby. we weighed the baby, measured her height an head circumference. My preceptor evaluated the baby and all exams was normal. As a student NP, it's something I rarely see with mothers under influence.
