The nurse is caring for Tom (preferred pronouns: he, him, his), an 84-year-old male client, who has experienced a brain stem injury. The nurse completes a nursing assessment including assessment of Tom's respiration rate, depth, and rhythm. Following the palpation of his radial pulse, 14 total respirations are counted in 1 minute. During this time, it observed that Tom's abdominal muscles tend to move with inhalation. At a couple of points in the observation, it is also noted that Tom took 2 very shallow breaths and then didn't take another breath for several seconds. He then returned to a regular rhythm for a short period of time and the cluster then repeated.
