I am working on an EBP paper and my nursing diagnosis is " imbalanced nutrition more than body weight" . After completing my community assessment, I found the members of the community consume a lot of fast food. I also found obesity, HTN, diabetes, and heart disease are prevalent in the community. The rate of obesity was highest than other diseases.

I am planning to organize a community outreach/education session. I need suggestions on interventions that could be appropriate for the identified problem (obesity) and how I can evaluate it.
