SCENARIO
Tumor lysis syndrome is defined as a, "potentially fatal complication that occurs spontaneously or more commonly following radiation, biotherapy, or chemotherapy-induced cell destruction of large or rapidly growing cancers such as leukemia, lymphoma, and small cell lung cancer" As a result of tumor intracellular contents, patients will experience rapid electrolyte imbalances and a severe increase ins serum uremia
While patients with TLS may appear differently, it is important for the nurse to monitor the patient for clinical manifestations, including dysrhythmias, seizures, hypertension, changes in levels of consciousness, anorexia, N/V, kidney injury, flank pain, and anuria. Nursing interventions include educating the patient for signs and symptoms to be on the lookout for that indicate electrolyte changes, administering medications such as diuretics, allopurinol, or sodium bicarb to correct disturbances; test urine pH, and implementing hemodialysis as indicated.
