This is an example of a SOAP note on the Head, Face, and Neck. Please follow the same format

Subjective

Denies history of frequent and severe headaches, dizziness and has never experienced any head injuries, neck pain, lumps. No surgeries to the head and neck

Objective

Examination to the skull indicates no abnormalities. Assessment of the face showed no issues presenting, face is symmetric and show no indications of involuntary movements. Patient has some acne on the face, but it is not severe. The color and texture of the skin is tan and smooth.

The neck is symmetric, and the patient did not have any issues with range of motion and did not have any neck pain when moving. Cervical muscles are strong and no abnormal pulsations were present. The thyroid, lymph nodes, and salivary glands were not enlarged and within the range.

Palpation of the lymph nodes resulted in the nodes were round, small, soft, and were free moving. Patient denied tenderness and inflammation was not observed. The trachea was symmetrical and the thyroid gland was easy to palpate and was not enlarged.

Assessment

Patient is healthy nature and only presents some facial acne.

Plan

Discuss the importance of cleanliness of face and Recommendation of better face wash for acne. Discuss how to check the lymph nodes and to notify a doctor if they are swelled and tender. Patient had no questions and understood the importance of face washing and lymph node palpating.
