Question

Lindsey Grey (Mannequin) Presenting Condition Lindsey was mending a fence out on his property at 08:00hrs this morning when he developed central chest tightness, which spread to involve his jaw and left arm, and was associated with SOB and diaphoresis. When it didn't completely settle with rest, Lindsey asked his wife to call 000. His symptoms also included feeling dizzy and becoming sweaty. In the emergency department he was given 300mg Aspirin, and 2 sprays of GTN, which has alleviated his pain a little. His ECG showed some sub-threshold ST changes, and his initial troponin (10:00hrs) was 23. Due to ongoing pain and ECG changes he was admitted under Cardiology, who are waiting for his serial troponin (due at 12:30hrs), and plan to start a GTN infusion. A preliminary diagnosis of unstable angina has been made, and a semi-elective angiogram is scheduled to happen later today. Past Medical History Lindsey has a history of high cholesterol and high blood pressure. A cigarette smoker 10-a- day. Regular Medications Atorvastatin 40mg tablet nocte Ramipril 5mg tablet daily Glyceryl Trinitrate (GTN) sublingual spray (400mcg/metered dose) prn Social History Lives with his wife. Has two sons who also work.
