The CQI Risk Management Project will highlight handoff tools (I-PASS) discussed in the Quality and Safety Literature Review through a PowerPoint presentation that outlines the Executive Summary, Issues, Data, and Action Plan with Timetable. The PPP would reflect what you would present to your institution's Quality and Safety Review Board. The goal is to apply the new knowledge of CQI to a real-world problem and how it should be addressed to improve healthcare delivery quality and safety standards.
1. Issue (handoff tools- I-PASS) to identify via a variety of sources (such as patient and other stakeholder complaints, providers, over- or underutilization, clinical quality or safety gaps and errors, administrative problems, changes in third-party payer requirements such as monitoring of 90-day discharge readmissions, changes in standards and regulations, accreditation concerns)
2. Identification of issues with the greatest impact on the selected population based on demographics, utilization and cost of care, and so on (use of prioritization)
3. Development or selection of measures/indicators related to the problem(s) (e.g., what will be measured), including thresholds
4. Description of data needed and data source(s) per measures/indicators
5. Detailed data collection plan, including identification of data collection methods
6. Identification of data collection barriers and if possible, resolution before data collection
7. Description of data analysis methods and who will collect data, training for data collectors, data recording methods to describe data, and timelines.
8. Ongoing evaluation of plan implementation to ensure work is done and the plan is effective in collecting and analyzing data.
