Assignment:

The Plan-Do-Study-Act (PDSA) cycle has been a long-standing favorite quality improvement tool in healthcare. You will recognize and appreciate the many aspects of planning a quality improvement initiative.

You are a QI professional with Baptist Memorial Hospital (BMH) in Memphis, TN. You have been asked to create a status report for an executive committee regarding a QI initiative focused on preventing hospital acquired conditions (HACs) for your large Medicare patient population. Specifically, you have been asked to target the issue of catheter-associated urinary tract infections (CAUTI). Your hospital employs the PDSA model for quality improvement and you have been asked to specifically address the "Plan" phase of the HAC initiative. Your report must include the following:

  1. Brief description of a generic PDSA cycle (describe the purpose and intended outcome of each phase, not specifically applied to your project).
  2. A rationale for the initiative (i.e., Why are we working to reduce HACs for our Medicare patients?).
  3. A rationale for where to focus your efforts (i.e., Why is CAUTI being targeted?).
  4. A description of the measure and its development (i.e., Where did the measure come from and how was it developed?).
  5. A critique of the costs and benefits of using a clinical practice guideline (i.e., Why use an existing guideline? Why not develop our own?).
  6. A description of the intended scope of implementation (i.e., Will the initiative be implemented in a particular unit(s) or throughout the entire facility?).
  7. A rationale for the intended scope of implementation (i.e., Why a particular unit(s) or why the entire facility?).

You have asked your assistant to provide some background information and she has given you the following:

  • Hospital Compare website (Medicare.gov) to compare BMH and Methodist Hospitals of Memphis, its primary competitor (the HAC results are found under the Complications & Deaths tab)
  • A clinical practice guideline containing steps to prevent CAUTI.
