Assignment

1. Either the topic of your Course Research Paper or the aviation accident you selected for your Course Case Study.

1) For the Course Research Paper that submission must include:

• The human factors in aviation safety topic and a draft title.
• A one-paragraph outline explaining how the paper is relevant to this course.

2) For the Course Case Study that submission must include:

• A clear identification of the accident (aircraft, date, location, and NTSB Final Report number) and a draft title.
• A one-paragraph outline explaining how the accident circumstances are relevant to this course.

2. Consider a work accident involving human error that you were either a part of or that you are aware of from your work. If you were not a part of or aware of such an accident from your work, interview a friend or relative about one they experienced. If possible, review an official report regarding the accident. Describe the circumstances of that accident in detail. Then analyze and evaluate that accident from all of the various human error perspectives (cognitive, ergonomic, behavioral, medical, psychosocial, and behavioral). Consider the root causes of the accident. Finally, propose methodologies for avoiding and/or managing such an accident in the future. As always, support your work with a reliable source(s).

3. applying the HFACS:

Once again go to the NTSB Web site and select one aviation accident report that particularly interests you. Review the Factual Information chapter (do not review the Analysis, Conclusions, or Recommendations chapters) of the Full Report (not the Summary). Then analyze and evaluate the human error aspects of the accident by creatively applying the data analysis tools of the Human Factors Analysis and Classification System (HFACS). Report your results and conclusions. As always, support your work with a reliable source(s).
