Client reports poor appetite over the last week and has been crushing their pills and mixing them with applesauce. Client reports nausea, vomiting, and difficulty sleeping for the past 24 hours. Client is alert and oriented to person, place, and time. States they are comfortable at rest, but they are still slightly nauseated and have developed a headache. No report of increasing dyspnea. Vital signs: temperature, 98.2°F (36.8°C); heart rate, 78 beats/min and regular; respiratory rate, 24 breaths/min; blood pressure, BP 128/72 mm Hg. Oxygen saturation is 94% on room air. Has cough productive with moderate amount of thick white sputum that client states is normal for them. Breath sounds decreased in lung bases bilaterally. No report of chest pain. S1 and S2 auscultated, no S3 noted. Strong radial and dorsalis pedis pulses palpated bilaterally. No peripheral edema or jugular venous distention noted. Active bowel sounds heard in all four quadrants. No report of blurry vision, haloes around objects, or distortion of color vision.
