A 67-year-old white male with a history significant for HTN, which is well-controlled with lisinopril 20mg PO once daily, presents to an outpatient acute care clinic (Urgent Care) complaining of a left-sided throbbing headache, intermittent loss of balance, and word-finding difficulties that preceded the headache that he first noticed upon awakening approximately 2 hours ago. His wife is present and is reporting that before going to sleep last night, he was "perfectly fine." The APRN recognizes that these symptoms are concerning, but the patient refuses to go to the emergency department at this time. The patient has a history of mild intermittent tension headaches, but this headache today is new/different for him. The patient does not have a reported history of falls/trauma and does not appear to have any injuries. The APRN proceeds to complete a comprehensive neurological exam to guide a differential diagnosis list further, educate the patient on the risks, and further guide diagnostic studies/medications/referrals (if necessary).
