Case Study: Collaboration and Communication
Cecilia is 34 years old and lives with her great Aunt Ruth (93 years old). Cecilia was born with Down Syndrome, and a moderate intellectual disability. With the support of loving family, Cecilia has lived a long and meaningful life. Cecilia started receiving support from disability service organisations 15 years ago. Support workers engage with Cecilia for 6 hours a day, 2 days a week
Unfortunately, Ruth health is declining. The treatment plan that she was following to treat chronic kidney disease is no longer effective and the doctor has informed her that dialysis will be required to sustain her life. This will mean that she will be in clinic for up to 5 hours per day, 3 times a week. Three long days, including travel to and from clinic, and being depleted of energy. Ruth feels that she will not be able to provide the best support for Cecilia.
The team is going to meet to discuss options that best suit Cecilia and Ruth.
The team consist of Cecilia, Ruth, their General Practitioners and other Allied Health Professionals, supervisors of the disability service provider, and you, the Support Worker
