Clinical Grade 04/07/2018 - cc: Gross hematuria and bladder mass. Pt had gross painless hematuria 4 days ago. PTA CT IVP showed a mass in the bladder w/ a superficial calcification. IMP: Bladder tumor, clinical findings are suspicious for a transitional carcinoma. Plan: Cystoscopy and TURBT. Scans 04/03/2018 - PTA CT IVP: 17 x 35 x 41 mm infiltrating mass in Lt posterolateral bladder wall w/ numerous associated calcifications. Findings are most suggestive of transitional cell carcinoma. No obvious extravesicular extension or mets. Bones negative. Normal LNs. Scopes 07/22/2018 - Cystoscopy, bladder barbotage: Hx of TCC. Urethra and prostatic urethra normal. Bladder w/ trabeculation and many small stones. No tumors. Operative Reports 04/10/2018 - Cystoscopy w/ TURBT of 3 - 4 cm papillary tumor (outpatient procedure): Thin-walled bladder w/ 3 - 4 cm papillary, non-invasive appearing lesion superolateral to the Lt ureteral orifice extending distally into the bladder neck. The tumor is broad-based and papillary-appearing primary tumor. Tumor did not appear to be muscle invasive as most of it was cold-looped off the surface of bladder. Area was then fulgurated. Mult calcifications in bladder. Urethra and prostatic urethra normal. Bimanual exam w/o any obvious mass.
