The client Mary Johnson (DOB 11/15/XX), a 36-year-old female, came to the OB clinic reporting headaches. She is 34 weeks gestation, gravida 1 (in vitro fertilization) para 0. She is a kindergarten schoolteacher. She is reporting severe headaches (7 out of 10 on the pain scale) for the past week. She does not report any vision changes, nausea and vomiting, or bleeding. No reports of seizures. Fetal heart rate is normal. Reflexes are normal. Urine sample has been provided per the provider's order and is located on the counter next to the sink.
