Provide written notice to the care recipient of the unspent amounts and confirm that it will be transferred to the new provider. Agree and document the agreed cessation date. Coordinate the cessation of services and commencement of new services with the new provider to ensure continuity of care for the care recipient. We will continue to provide care and services to the care recipient until the date they commence services with the new provider. Facilitate a safe and streamlined handover of information to the care recipient's new home care provider. Submit notification of the cessation date through the Aged Care Provider Portal within 31 days of care ceasing. Ensure continuing care recipients are aware of their right to retain their pre-1 July fee arrangements and the steps they must take to enact that wish. Retain records of the written notice to the care recipient of unspent home care amounts and records relating to the payment of the unspent home care amount to the new provider.
