A 58 year old woman is admitted with myasthenia gravis. Her condition has deteriorated and she is in crisis and now requires mechanical ventilation. She continues to be alert but unable to perform any self care. Her grandson who lives with her has been caring for her and states "I help grandma into her wheelchair but she did everything else. I don't know what I am going to do. I promised I would always take care of her." In order to plan your intervention with your patient and her family, you consider the following:
