Mr. Walker is a 48-year-old man being discharged from the medical-surgical unit today after having a large myocardial infarction. He will be accompanied home by his wife. You are the discharge nurse today. After reviewing his medical record provided you will complete the following tasks. The project will be guided based off of the medical, surgical, and social history.

1. Write out appropriate discharge instructions including but not limited to medical diagnosis, lifestyle changes, and diet.

2. List medications Mr. Walker may be discharged home with and education for each medication listed.

Past Medical History:

· coronary artery disease

· hypertension

· gastroesophageal reflux disease

· chronic allergic rhinitis

· mixed hyperlipidemia

· chronic obstructive pulmonary disease

· No known drug allergies

· Obesity with BMI of 31

Past Surgical History:

· tonsillectomy

· cardiac catherization with 3 stents placed

Social History:

· smoker with 2 packs per day history X 24 years

· drink 1-2 beers per day
