This is the third admission in six weeks for 46 year old Gray who is developmentally delayed and living in a training center. each admission diagnosis has been for dehydration acute renal failure thrombocytopenia anemia and mental status changes. his history also includes seizures. during the first two admissions dehydration, renal failure, and mental status changes returned to normal by the time of discharge. it was believed that his seizure medication was causing the thrombocytopenia, and it was changed to another agent. still his problems returned and compounded. Gray has been independent in self-care and living in a cottage on the center's grounds with other high functioning persons. because his condition began to deteriorate he was moved to the center itself where more care could be provided. the training center attendance reported that Gray's condition had been going downhill for the past four months. recently he started refusing food and he was losing a considerable amount of weight. when questioned the attendants said they believed this behavior could be related to a temper.
