When writing the goal, consider the nutrition problem you have identified: - Mr. Albertson is a 39-year-old male with an end stage renal disease. He was seen in the clinic after developing a fever, chills, general malaise, decreased appetite, midsternal chest pain, left arm AV graft thrombus formation with thrombectomy, AV graft infected with MRSA and shortness of breath. NFPE findings are normal. Patients have past medical history of IDDM, HTN, and receiving hemodialysis due to ESRD. Past social history of alcohol, smoking and H/O IVDA. Patient also has family history (father) of coronary artery disease. The patient's current weight is 137 lbs and has a BMI of 19.1 (Normal). He had a history of 10 lbs. of weight loss from the past month which is 6% of his UBW due to hypermetabolism. Patients at risk' for severe malnutrition secondary to intake and 10 lbs wt. loss. Vitals show 172/86 blood pressure indicates stage 2 high blood pressure. Lab results findings show elevated Glucose, BUN, Creatinine, potassium which supports (ESRD and IDDM). Low calcium, chloride, hemoglobin, and hematocrit support ESRD and high phosphate (Hyperphosphatemia). elevated PTH level.
