Case Study: A 16-year-old Hispanic male presents with his father for a follow-upvisit. The father reports that the primary concern is the patient experiencing "thoughts of killing himself."Subjective: History of Present Illness (HOPI) Patient presents for follow-up for medicationmanagement and has been taking Fluoxetine 10 mg since being prescribed at his initial evaluation. Hehas been working on the CBT skills assigned to him during therapy sessions, and he feels thesesessions are helpful to him. Over the past week, the patient have started to have thoughts of drivinga car off the road. His father took him to the Emergency Department last night, and thepsychiatrist did not admit him after evaluation and encouraged him to come to this follow-upappointment. The patient reports that initially, he was feeling more energy and feeling a little betterwith the medication. He still feels down; however, he is having more interest in doing activitiesHe enjoys it, is sleeping better, and his appetite has improved.
