Daisy Parker, a 72-year-old female client, is admitted on telemetry monitoring with severe aortic stenosis. She has a history of a congenital defect of the aortic valve and a recent anterior wall myocardial infarction 6 weeks ago. Her current medications consist of digoxin 0.125 mg every day; atenolol 50 mg every day; lisinopril 10 mg every day; furosemide (Lasix) 40 mg every day; potassium chloride 10 mEq, one tablet every day; rosuvastatin (Crestor) 20 mg every day. She is on a cardiac diet with a 2-g sodium restriction. The nurse assesses the client, noting the vital signs: T, 98° F; BP, 150/80 mm Hg; HR, 80 beats/minute; RR, 24 breaths/minute. The client has an aortic systolic murmur heard after the S1, and the point of maximal impulse is at the 6th intercostal space (ICP). The lungs are clear anteriorly and posteriorly. She has no chest pain or discomfort at rest but stated that with any activity she is extremely short of breath, has chest pain, feels dizzy, and at times passes out. The client is scheduled in the morning for a transcatheter aortic valve implantation (TAVI). The LPN/LVN needs to determine necessary nursing management for the client.
