Client found on bathroom floor by daughter who called 911. EMS arrived and observed blood stains on clients clothing and in the bathroom. Client presents to ED with confusion and lethargy. daughter present and is historein fo the client.
Social history Client is recently widow(1 year ago) and is retired. daughter states client does not smoke, ingest alcohol, or use illegal drugs. Daughter states client lives alone but that the family and neighbors visit the client daily, especially since the loss of her husband a year ago.
Past medical history: Osteaarthritis-seeing orthopedic surgeon every 6 months. started on 400 mg ibuprofen orally TID three months ago. also started on a PPI at that time for gastric mucosal protection. daughter did not recall the name of the medications.
Major depression: Evaluated and screened for depression 6 months ago, as well as possible dementia. started on setraline 25mg orally daily.
Home medications:
Ibuprofen 400 orally TID, Setraline 25mg oray daily, acetaminophen, asprin,
Vital signs as follow: B/P80/50, HR 120 Sinus tachycardia, RR26, Spo2 90% on room air. neuro: Oriented to person and place, moaning, lethargic, confusion. asking for her dead husband, but easily reoriented. aurousable to touch.
