Assignment:
I have to complete medication cards for the following patient:
2-year-old patient who is 36 lbs (16.4 kg). The child has been admitted for Community Acquired Pneumonia (CAP) and respiratory distress. The child also has a history of GERD and cyclic vomiting syndrome.
For the following drugs I must understand safe dose calculation and answer if the dosage ordered is safe. If I could please get some guidance on how to properly do these calculations, I would appreciate the help.
- Tylenol: 240 mg PO Q6 hours
- Motrin: 165 mg PO Q6 hours
- Cefdinir: 230 MG PO Q day
- Prednisolone: 16.4 MG PO BID
For The IV meds I must complete the following calculations
- Ceftriaxone: 850 mg IV Q24 hours
- Ondansetron: 4 MG IV Q 4-6 hours
- Famotidine: 4.1 mg IV Q 12 hours
1. What is the minimum concentration for the medication?
2. Based on the minimum concentration, how many ml's of the drug should come from pharmacy (your volume)? This is the main part I am confused on.
3. What is the infusion time of the drug? For Ceftriaxone: 850 mg IV Q24 hours am I correct in indicating the answer as 24 hours?
4.What do you expect the rate of the infusion to be (in ml/hr except for IV push meds)? I believe this should be my formula but I am not confident.
