Vignette: Caleb is a 12-year-old male who presented to your office for an evaluation.

He has a medical history of chromosomal abnormality at birth, resulting in significant cognitive delays and deficits. Caleb's Child Study Team at school has recently classified Caleb as having Specific Learning Disability in Reading, as well as a Specific Learning Disability in Math, and he receives group special education tutoring services for one hour per day in school. Caleb does not receive any services at home or in the community. His parents requested that you evaluate him due to concerns that Caleb is misdiagnosed and/or is not receiving appropriate services.

Upon completing your evaluation, you determine that Caleb has an IQ of 58. He has significant delays in his ability to care for himself and needs substantial support in areas such as basic hygiene and self-care (e.g., requiring assistance with cleaning himself after using the bathroom, requiring oversight to use a microwave). You also find that Caleb is functioning at a first-grade level across all major academic subjects, including reading and math, despite being in the seventh grade.
