Assignment:

Faddey Dostoevsky is a 40 year old patient of Stonemount Medical Center. His Primary Care Provide, Dr. Wacko Belly has known him since infancy and has a complete mastery of his health records. Dr. Belly is able to tell what the outcome of Faddey will be with any type of prescription. Faddy leaves about 10 miles away from the health center and sometimes when he is not able to make to his PCP's office, he uses the telehealth app on his phone to meet with his doctor for checkups. Stonemount has attained Stage 3 of meaningful use, which means they harness every aspect of of healthcare IT in their care processes, including interactive patient care portals. In the previous year, they were rated the most preferred medical facility by most patients.

After Faddey's telehealth appointment one day, he left his phone on the dinning table and went up to is bedroom for about 5 minutes after opening the door for his friend who came to visit after his appointment. A week later his employer asked him about a condition he had not discussed with anyone apart from his provider. Not knowing how the information was made available, he asked his PCP if his information was requested or shared with anyone, but his PCP confirmed no information was requested about him nor shared with anyone.

Faddey is still asking to know how his information was released/leaked till date.
