Case study

Jake has been larger than most kids from the day he was born. Now, at age 10, he towers over all the other boys and is 15 pounds heavier than most, with most of the extra weight being muscle. Over the last couple of years, Jake's parents encouraged him to channel his size and energies into sports, but he wasn't very coordinated and was teased by other kids, who called him "the gorilla." When this happened, Jake would sometimes lash out at the child who teased him, punching or kicking him. Given Jake's size advantage, this behavior usually resulted in moderate to severe injury for the other child. In the last 6 months Jake has been ambushing children who have been mean to him in the past on their way home from school and beating them up fairly severely. This has accomplished Jake's goals-the retribution against the other children feels good, and he doesn't get teased much anymore. Lately he's taken to ambushing children who have never teased him, just to send a message to others to stay clear of him and treat him with respect.

  • What diagnosis would you give Jake?
  • List the criteria from the DSM-5 that you believe Jake meets and match a behavior identified in the case study to each criteria you identify from the DSM-5.
  • As Jake's counselor, what evidenced-based interventions would you use with Jake?
  • Why would you use the interventions you identified?
  • Discuss 3 things you can do to help build rapport with Jake and increase the chances of him being open and forthcoming with you in therapy.
