HPI: Mr. W is a 56 y.o. man who presented to the E.D. with c/o moderate abdominal pain that he describes as continuous, for the last two days. He reports that it started in his epigastric area but has now begun to radiate through to his back. This morning he had an episode of N/V which prompted his visit to the E.D.
1. Based on his initial presentation and complaints, what are your leading differential diagnosis? List your top three with your rationale
VS: ST 103 RR 18 99.9 138/64
PMH: HTN, Hypertriglyceridemia, Choledocholithiasis, Back Pain
Social Hx: Never smoker, no illicit drugs, walks daily, Reports no ETOH during the week but likes "to let loose on the weekends".
Meds: Lasix 20 mg po bid, Oxycodone 5 mg Q8H prn pain
