Medical Record 17-years-old femaleis here for sick visit visit with mother who provided history. Chief Complaint: Patient presents with Vaginal Itching Vaginal Discharge HPI: As per pt having vaginal itching and having white vaginal discharge for 2 days no other sx. Denies any fevers/N/V/dysuria/any urinary sx or any vaginal bleeding/vaginal lesions/abd pain. Sexually active with males and says uses condoms on and off. Has Hx of STD Chlamydia ~ a year ago with similar c/o vaginal itching. Review of Systems: General/Constitutional:denies any fevers, chills, headaches, dizziness, fatigue etc HEENT: Denies any nasal congestion, runny nose, sore throat, eye redness or discharge, ear symptoms, throat or neck pain or swellings etc. Lungs: Denies any cough, wheeze, stridor, SOB. Heart: Denies any chest pain, palpitations. Liver/Spleen/Kidneys: Neg Gastrointestinal: Denies any N/V/D/abd pain/any distension or any lumps. Genitourinary: Denies any urinary sx like dysuria, frequency, urgency, discoloration of urine, incontinence. No menstrual problems. Neurologic: Denies any weakness, numbness or any focal deficits, changes in gait. Skin: Denies any rashes Musculoskeletal: Denies any joint pains, muscle aches, limited ROM anywhere etc. Endocrine: Denies any polyuria/polydipsia Allergy/Immunology: Negative. Psychiatry: Negative, no changes in behavior etc.
